The medical document in the form of a card that was to be filled in by patients visiting the Hi Red Centre mobile health clinic which only visited streets starting with V, T. R or E (vTRE). There is a paper variant of this item but this design by Maciunas has an appropriated image of a baby as part of a photocollage on the front. VG+. Very scarce.
